Note:  The same form is used for access to Query Manager and for dataLink.  dataLink Developers need only obtain a signature from their IT Director.  They do not need the reporting lead's signature.  

dataLink Developers who will access Query Manager as well will need both signatures but will only need to send one form.  Just inform the recipient of the form that it is being used for both dataLink and Query Manager access.

Electronic signatures are acceptable.
