Note:  The same form is used for access to Query Manager and for dataLink.  Query Developers need only obtain a signature from their reporting lead.  They do not need the IT Director's signature.  

Query Developers who will access dataLink as well will need both signatures but will only need to send one form.  Just inform the recipient of the form that it is being used for both dataLink and Query Manager access.

Electronic signatures are acceptable.
