Student Questionnaire (HCX)

This Quick Reference Guide (QRG) has been prepared to support the upcoming User Acceptance Testing (UAT) activities with our colleges. Please note that this functionality is not yet available in the production environment.

Purpose: Use this document as a reference to access, complete, and submit the Mobile (HCX) Student Questionnaire.

Audience:  Students.

The appearance of college mobile applications may vary across institutions. This guide provides a general overview; however, your college's app may feature unique images and options.

  1. On the Mobile (HCX) Springboard page, select the ctcLink Login tile.  
  2. The ctcLink Login page displays.
  3. Enter your ctcLink ID.
  4. Enter your Password.
  5. Select the Sign In button. The Okta Account section of the ctcLink Reference Center provides helpful information on accessing your ctcLink account.

Option 1: Directly from the Dashboard

  1. Look for the Tasks widget on the main Dashboard. It is located on the right-hand side of the screen.
  2. Find the Student Questionnaire task listed under the Tasks section.
  3. Click directly on the Student Questionnaire task to open and complete it.

  1. The Student Questionnaire page displays. The following steps will guide you through answering the questions about educational benefits and support services. Your responses will help your college meet state and federal reporting requirements and provide support tailored to your needs.

Instructions

Step 1. Select Next to enter responses in the Student Questionnaire.

Step 2. Enter responses and select Confirm. Select Next to proceed to the final step.

Step 3. Select Complete. The task is complete and will be removed from the To Do list.

  1. Tooltips (a small circular symbol with the letter "i" inside, which stands for "information") provide additional helpful information when completing the Student Questionnaire.
    • If you're using a computer and a web browser, simply hover over the tooltip icon with your mouse to view the details.
    • On a mobile touchscreen device, press and hold the tooltip icon to access the same information.
  2. Process complete.

Option 2: Using the Left-Hand Navigation Menu

  1. On the left-hand navigation menu, locate the section titled "My Information" (with a numbered indicator next to it).
  2. Click on "Tasks" under the My Information section.
  3. The Tasks section will display a list of items that require your attention.
  4. Locate the Student Questionnaire task.
  5. Click on the Student Questionnaire to open and complete it.
