Student Questionnaire (HCX)
⚠️This QRG reflects future functionality that will be implemented on 07/22/2025.
Purpose: Use this document as a reference to access, complete, and submit the Mobile (HCX) Student Questionnaire.
Audience: Students.
The appearance of college mobile applications may vary across institutions. This guide provides a general overview; however, your college's app may feature unique images and options.
- On the Mobile (HCX) Springboard page, select the ctcLink Login tile.
- The ctcLink Login page displays.
- Enter your ctcLink ID.
- Enter your Password.
- Select the Sign In button. The Okta Account section of the ctcLink Reference Center provides helpful information on accessing your ctcLink account.

Option 1: Directly from the Dashboard
- Look for the Holds widget on the main Dashboard. It is located on the right-hand side of the screen.
- Find the Student Questionnaire listed under the Holds section. Click on it to view Details and Instructions.
- The Instructions display:

Educational benefits and support services may be available to you. You will be asked to complete a short questionnaire each academic year with the option to update your responses more frequently as your circumstances change. Your responses may be shared with and used by each Washington Community and Technical College where you have an admissions record and utilized to help meet state and federal reporting requirements.
Select the link below to respond to the Student Questionnaire. The Student Questionnaire is optional, and this hold does not prevent you from enrolling or completing any self-service tasks. This hold will be removed from your account within one business day of submitting responses. If you receive the same hold for more than one college, you only need to respond once.
- Click the Start the Questionnaire button to access the questionnaire.
- The Student Questionnaire page displays.
- Answer each required question. Questions marked with an asterisk (*) are required and must be answered before you can save the form.
- Click "Save" after completing all questions to submit your responses.

Tooltips (a small circular symbol with the letter "i" inside, which stands for "information") provide additional helpful information when completing the Student Questionnaire.
- If you're using a computer and a web browser, hover over the tooltip icon with your mouse to view the details.
- On a mobile touchscreen device, press and hold the tooltip icon to access the same information.
- Process complete.
Option 2: Using the Left-Hand Navigation Menu
- On the left-hand navigation menu, locate the section titled "My Information" (with a numbered indicator next to it).
- Click on "Holds" under the My Information section.
- The Holds section will display a list of items that require your attention.
- Locate the Student Questionnaire and click to open and complete it.
- The Student Questionnaire page displays.
- Answer each required question. Questions marked with an asterisk (*) are required and must be answered before you can save the form.
- Click "Save" after completing all questions to submit your responses.
- A message appears on the screen stating, "Your information has been updated."
