Student Questionnaire (Staff) - Entering Responses on Behalf of Students
⚠️This QRG reflects future functionality that will be implemented on 07/22/2025.
Purpose: Use this document to access the Student Questionnaire (Staff) page, and submit the Student Questionnaire on behalf of the student.
Audience: Campus Community
You must have at least one of these local college-managed security roles:
- ZZ CC Student Questionnaire
Navigation: Campus Community > CTC Custom > Student Questionnaire Form > Student Questionnaire (Staff)
- EMPL ID: Enter the student’s EMPL ID into the designated field.

Academic Year:
- Click the lookup icon 🔍 next to the Academic Year field.
- In the Lookup window, select the appropriate Academic Year (e.g., 2025 - Summer 2025 to Spring 2026).
- You will return to the main screen with both fields populated.
- Between the last Census Date for spring term and the Begin Date for summer term, staff will have the option to select either the current or future Academic Year questionnaire. This will give staff the option to enter a questionnaire for students attending the current Academic Year or enter a questionnaire for students who are enrolling in the upcoming Academic Year beginning in summer.
- Click the Add button. This will open the full Student Questionnaire (Staff) form for the selected student and academic year.
- Review and complete/update all required questions. Questions marked with an asterisk (*) are mandatory and must be completed before saving.
- Use tooltips for guidance. Click the information icon ⓘ next to each question for more details. These tooltips provide clarification to help accurately complete the form.
- Once all required fields are completed, click the Save button at the bottom of the form to submit the student’s questionnaire.
- To enter or update responses for another student:
- Click the Add button (located to the right of the Save button) to return to the search screen.
- Repeat the steps above.
- When an original response on the Student Questionnaire has been modified, a double parallel line icon (≡) appears next to the question.
- Tooltip Message: Hovering over this icon will display the message: "Changed response."
- This visual indicator helps staff quickly identify updated responses during data entry or review.

Overview
A pre-designed template is available for colleges interested in creating a 3C or Message Center communication. This "global template" includes a single merge field for the student’s name and a designated section for college-specific contact information. Colleges may choose to adopt and customize the template; however, additional set up related to changing template text or merge fields must be completed by the college when implementing a new 3Cs or Message Center.

Support and Setup by CS Core Support
Upon ticket submission, CS Core Support will configure the college’s 3C Communication or Message Center using the original "global" version of the template. This setup includes the standard live fields. Yellow highlights in the template indicate areas where a name live field is needed or where content must be completed by the college. Any additional modifications to the global template—such as adding more live fields—are the responsibility of the college to configure and test.
Ticket Submission Instructions
To request configuration of the communication, colleges must submit a ticket with "ER 232 Communication Request" in the subject line to ensure proper routing.
Colleges should include an updated version of the global template that reflects their preferred contact information or other text edits. Please note that SBCTC will only configure the name field in the salutation as a live field; any additional live fields must be added and tested by the college.
